Medical Examination for U.S. Immigration: Information for Patients

What the Medical Examination Includes

  • Review of your medical and vaccination history.

  • Mandatory lab tests for tuberculosis, gonorrhea, and syphilis (depending on your age).

  • Required vaccinations determined based on age and other factors.

  • COVID-19 vaccination proof is required for applicants aged 6 months and over.

  • The process takes approximately 1 hour.

Cost of the Medical Examination

  • $350: Includes the doctor’s visit and all mandatory tests (vaccines are not included).

  • Additional costs:

    • Blood draw for immunity check (optional) - $65 (or you can use your health insurance, but coverage is not guaranteed).

Additional Information

  1. Influenza Vaccine Requirement: From September through March, all patients must receive the flu vaccine.

  2. I-693 Form Reissuance Fee: $75 if the form needs to be reissued (e.g., if the sealed envelope is opened or USCIS did not receive it).

  3. No Follow-Up for Form Collection: You do not need a follow-up visit just to collect the completed I-693 form.

  4. Follow-Up Visit for Treatment: If treatment is required (e.g., for syphilis), a follow-up visit fee of $250 applies. Alternatively, you may receive treatment from your primary care physician. If we accept your insurance, we can bill the insurance.

  5. Insured Applicants: Can receive vaccines during the form collection visit, and may submit this to health insurance (vaccines cannot be submitted during the initial visit).

  6. Vaccinations from Local Pharmacies: We accept vaccine records from local pharmacies or vaccination centers.

  7. Vaccination Requirements Vary by Age: Proof of vaccination can be provided through previous records or lab testing (blood titers available for adults only).

What to Bring to the Medical Examination

  • Identification: Passport or driver's license.

  • Vaccination Records (including your COVID-19 vaccination card).

  • A registration form, which can be filled out at our office.

Medical Results

  • Medical reports are usually ready within 5-10 working days after your appointment.

  • You will receive your Form I-693 in a sealed envelope, which should not be opened.

Additional FAQs

  • Fasting for Blood Tests: Not required before your appointment.

  • Using Previous Medical Exam Results: Only exams performed by a USCIS-authorized Civil Surgeon for immigration purposes are accepted.
